Before filling out the Special Events Form, please read the Special Event Form Instructions to gain an understanding of how to complete the form & the processes and requirements that are the applicant's responsibility by filling out this form.
Special Event Form:
Once you have read the instructions above, please complete a Special Event Form. To submit the form, please do ONE of the three methods:
1. Please save the completed form to your hard drive. Right click the Special Event Form link above & select "Save As Target". Save the file in a known location & fill out the appropriate information. Finally, email it as an attachment to Kerrie Peltier at the Parking Office.

Please note: Special Event Forms MUST be submitted 2 weeks before the proposed event is to occur.
